Please choose the appropriate form for the correct camp your camper will be attending.

Once you have completed the application packet in it’s entirety and have had a physician sign off on the appropriate forms, please email us a copy of these documents to director@campemanuelohio. 

You can hand in these forms on the first day of camp during check-in as well.
